Getting started is simpler than you'd think

    We match services to the person's goals and coordinate with your support plan and funding source.

    Your support professional begins, and we stay in touch to keep things working.

Eligibility & funding

Programs we accept

We're an approved provider across Florida's home and community-based programs, so support can be authorized through the plan you already have.

Who we generally support

  • Adults age 18 and older with a disability, living in the Central and Suncoast Region of Florida.
  • Enrolled in a Florida Medicaid waiver or managed care long-term care program, or working toward one.
  • Support needs identified in a current support plan, or a situation you'd like help assessing.
  • Family caregivers who need planned or emergency respite coverage.
