Eligibility & funding
Programs we accept
We're an approved provider across Florida's home and community-based programs, so support can be authorized through the plan you already have.
HCBS Medicaid Waiver Provider
We are an approved Home and Community-Based Services (HCBS) Medicaid Waiver provider, so waiver-funded supports can be delivered in your own home and community rather than in a facility.

Who we generally support
- Adults age 18 and older with a disability, living in the Central and Suncoast Region of Florida.
- Enrolled in a Florida Medicaid waiver or managed care long-term care program, or working toward one.
- Support needs identified in a current support plan, or a situation you'd like help assessing.
- Family caregivers who need planned or emergency respite coverage.
