Eligibility & funding

Programs we accept

We're an approved provider across Florida's home and community-based programs, so support can be authorized through the plan you already have.

Who we generally support

  • Adults age 18 and older with a disability, living in the Central and Suncoast Region of Florida.
  • Enrolled in a Florida Medicaid waiver or managed care long-term care program, or working toward one.
  • Support needs identified in a current support plan, or a situation you'd like help assessing.
  • Family caregivers who need planned or emergency respite coverage.
